Touring the Menagerie of Facial Fillers
Injectable compounds account for the vast majority of cosmetic treatments performed in the United States, with Botox alone accounting for nearly half of the overall total of 11 million procedures last year. Although Botox is well known by name, many injectable dermal fillers that are available are completely unknown to most people. This article will give a basic primer on the different compounds, their effects, their disadvantages, and their benefits.
Acid Breakthrough
The current surge in interest in new injectable fillers began once collagen, the initial filler used in the early days of the mass-market plastic surgery revolution, was challenged by a new set of injectable fillers that were made from a naturally-derived product, but did not require allergy testing, had very low incidence of side effects, and lasted at least 33% longer than the older filler. In addition, people do not need an allergy test prior to taking hyaluronic acid, as they did with collagen.
Hyaluronic acid is a naturally-occurring compound in the body. It is a component of the skin's natural moisture, and the fluid lubricating joints and the spine.
Hyaluronic acid compounds are used for treating marionette lines and wrinkles around the mouth, and, sometimes, frown lines in the forehead. The effects of most hyaluronic acid compounds reliably last about six months, although some studies indicate occasional results of a year or more.
Hyaluronic acid fillers are sold under the brand names Restylane, Juvéderm, and Perlane.
Body, Heal Thyself!
Sculptra is different from other fillers. Instead of just filling your wrinkles, it produces an irritation reaction that stimulates your body to produce its own filler. Sculptra's base material, poly-L-lactic acid, is a chemical variant of the substance that causes the burn in your muscles during exercising. Sculptra treatments do not require prior allergy testing.
Sculptra is not used as often for concentrated wrinkles, but more often for large areas of the face, and is increasingly taking the place of autologous fat transplantation, which can be more uncertain. Sculptra is often used for:
• Skin folds
• Sunken eyes
• Cheek augmentation
• Chin augmentation
Sculptra is ideal for correcting excess fat loss in the face as a result of working out or dieting. Some patients see results lasting for two years or more. Because it depends on your body's response to the injections, you will see gradual improvement, and more than one session (probably 3) is required to achieve the desired results. In addition, make sure your doctor has experience working with the compound because otherwise results can be uncertain.
Advanced Composites
One of the most recent innovations in injectable fillers is the introduction of compound materials that consist of a carrier gel and microspheres. The basic theory of these compounds is that the gel will be absorbed by the body along the normal time frame for collagen or hyaluronic acid compounds, say about four to six months. However, the microspheres are made of more durable material, material that resists digestion by the body. The theory is that the body will fill in around the spheres with its own material, especially collagen. The main difference between the two versions of this principle is the composition of the microspheres.
Bone Filler
Radiesse was the first of this style of injectable designed. Initially marketed under the trade name "Radiance," it uses microspheres composed of calcium hydroxyapatite, a material originally designed as a bone filler. This material is a naturally occurring one in the body, and as a result it is digested by the body over time. Clinical trials indicate that this filler can give results lasting over a year.
Permanent as Plastic
ArteFill is a relatively new filler, approved for use in the United State in late 2006. It purports to be a permanent filler, because it uses undigestable polymethilmethacrylate (PMMA) microspheres. PMMA is commonly known under names such as plexiglass or Lucite. Theoretically, the use of indigestible plastic microspheres could lead to a permanent improvement of facial wrinkles. However, the technology has only been under development since 2004, and has only been approved for use in the United States for a short time, and so far it has not been used on as large a scale as other fillers, so no one knows for sure just how long the results might last. One study conducted prior to FDA approval showed that the results of ArteFill did not diminish at All Over the first six months following treatment, i.e. the effective lifetime of Restylane or Juvéderm.
The filler that is best for you will depend on what results you are seeking, both in terms of what problems you are seeking to correct and the frequency of retreatment you desire. And, of course, cost is an issue as well. In order to make your decision most effectively, consult with your plastic surgeon.
